  • Use of this website and purchase of these services constitutes agreement to the following terms and conditions.

  • Users must be 18 years of age or be represented by a parent or guardian to sign up to receive services.

  • This website provides the ability to sign up for and pay for nutrition counseling services. Information provided on the website or in the counseling sessions is not considered professional medical advice, nor is it intended to diagnose or treat any illness.

  • You should always consult with your doctor about your health concerns and before seeking outside therapies. Consult with your doctor before making any changes to your medication and/or supplements and before making drastic diet and lifestyle changes.

  • Users of the website are not considered clients of Blaser Health until they have reviewed, signed, and returned the informed consent disclaimer, and provided all requested information.

  • Any harmful or fraudulent use of this website will result in immediate termination of use of the website and access to all services. Do not download images or content found on this site.

  • I reserve the right to change and update these terms at any time without notice. Date of most recent changes will be posted at the top of the page.

  • I reserve the right to refuse service, at my discretion.

  • You as a client commit to providing truthful information about your health, diet, and lifestyle so we can best formulate a plan to suit your personal needs.

  • Please refer to our Privacy Policy to see how information is handled.

  • I do not make any claims, warranties, nor guarantee results, while providing counseling services.

  • There are no guarantees or claims that you will experience the same results as other clients.

  • By purchasing services, you release Blaser Health and Laurel Blaser from any claim or legal liability.

  • I will do what I can to ensure client satisfaction (within reason). Refunds will not be issued for counseling sessions and services that have already taken place. If you have questions or concerns, contact laurel@blaserhealth.com.

  • All services are non-refundable unless:

    • Blaser Health cancels session due to unforeseen or emergency scheduling conflict. Session will either be refunded or rescheduled at no additional cost.

    • Blaser Health is unable to provide the client services due to severity of health condition.

  • Blaser Health has a 48-hour cancellation/rescheduling policy. Clients will be charged $40 for a missed appointment rescheduling fee if proper notice is not provided to Blaser Health by phone or email.

  • These Terms and Conditions, and any dispute arising out of it, shall be governed by the laws of the State of Oregon.

  • If you have a complaint or should issue arise with the use of Blaser Health’s nutrition counseling services, contact me directly at laurel@blaserhealth.com. If we are unable to resolve our dispute amicably, you agree that we shall submit our dispute to binding arbitration with the American Arbitration Association. Arbitration hearing shall be held within 50 miles of Portland, Oregon or Eugene, Oregon. Arbitration judgements are binding, and further punitive or consequential damages will not be pursued.
